Plan of Care for Stroke Patients (PART 2)
When your loved one who suffered a stroke is already allowed to leave the hospital or rehabilitation center, there are some things you can do to help him or her ease back to normal living. A plan of care is a systematic process which involves health-related disciplines from the patient’s doctor or health provider. Plan of care can be a key to your loved one’s readjustment back to his or her everyday life. Plan of care must start long before a patient is ready to leave the hospital or rehabilitation center. It includes an assessment of the patient’s living environment, home health and family support, disability entitlements and potential rehabilitation plans. The patient, the family and the home health staff can work together to help the patient fully enjoy the benefits of the program. A plan of care’s goals includes:
- Making sure the patient lives in a secure and safe place
- Deciding what type of home health care, assistance or special equipment is needed by the patient
- Arranging rehabilitation programs and services at home, this may include daily monitoring by a home health aide
- Choosing a doctor or health care provider who will continuously monitor the patient’s health and medical needs
- Teaching and guiding the patient’s family important skills such as daily care and assistance at home
- Helping the patient look for fruitful activities such as employment opportunities, volunteer acts and even daily skills such as driving a car (only if the patient is able and interested to do so)
- Discussing any sexual concerns the patient may potentially have
Rehabilitation is an important part of recovering from a stroke. Through rehabilitation, stroke patients relearn and regain basic skills such as speaking, eating, dressing, and walking. The goal is to improve function so you can become as independent as possible.
